Patients can login to Eye Associates website initially using their MRN(medical record number) and mobile number. Enter MRN in place of
User Id and mobile number given at the time of appointment in place of password.
After getting login to Eye Associates website, goto Home page. Multiple visits numbers will be shown on right side of page. Click on the desired visit number and you will get detailed visits information in the form of Clinical Charts and Dictated Notes.
Clinical Chart contians detailed visit information of patient while Dictated Note contains breif summary of patients disease including symptoms and diagnosis.
To get number of glasses prescribed to patient, login to Eye Associates website and open Clinical chart or Dictated Notes and detailed glasses information will be mentioned there.
